WHAT TO BRING: All completed forms from the documents page. School Health form- front page completed and signed by parents, back page completed by physician. If new, original birth certificate and payment. You can bring a check or cash if you want to purchase a school shirt. You will also bring your school supply that is listed on this website. We meet in the church sanctuary and enter at the side door where you see a long wheel chair ramp.
